From the first point of contact to long-term housing stability, the Lancaster County Homelessness Coalition brings partners together to provide a continuum of support. Our network connects people with services that meet immediate needs, create pathways to housing, and help build lasting stability.
Our network supports programs focused on:
- Connecting People to Help — Street Outreach & Coordinated Entry
- Meeting Immediate Needs — Emergency Shelter, Day Centers/Day Shelters & Crisis Housing
- Creating Pathways Home — Pathways & Permanent Supportive Housing (PSH)
- Preventing Homelessness — Homelessness Prevention & Diversion
- Responding to Emerging Needs — Special Projects Defined by HUD
Together, these programs form a coordinated network designed to help people move from crisis toward safe, stable housing and the opportunity to thrive.
About the Homelessness Coalition
The Lancaster County Homelessness Coalition brings together 75+ organizations including health and human service providers, private-sector partners, and faith-based organizations with a shared vision:
Building a community where homelessness is rare, brief, and non-recurring.
Working across Lancaster City and County, the Coalition connects people, organizations, and resources to address homelessness from every angle. Through service coordination, advocacy, public education, and community organizing, we work together to respond to immediate needs while building stronger, lasting housing solutions.
Leading Through Collaboration
The Coalition is guided by a Governance Board that provides community-wide leadership and direction. The board brings together diverse perspectives from across Lancaster County, including municipal, private, nonprofit, education, and faith-based organizations, as well as individuals with lived expertise of homelessness.
Our Role in Action for Homelessness
The Office for the Lancaster County Homelessness Coalition (O4C) serves as the lead agency for PA-510, coordinating the community's Continuum of Care (CoC) efforts and supporting a collaborative system of housing and services.
As the lead agency, O4C oversees CoC policies and procedures and CoC project funding decisions.
O4C also serves as:
- Collaborative Applicant for the CoC Program Competition
- HMIS Lead for the Homelessness Management Information System
- Coordinated Entry Lead Agency for PA-510
O4C reports to the Governance Board on a bi-monthly basis, providing updates on the current state of homelessness, available services, emerging needs, and opportunities to strengthen the community's response.
What Is a Continuum of Care?
A Continuum of Care (CoC) is a local or regional planning and coordination body that brings together organizations and community partners to address homelessness. This includes nonprofit and housing providers, victim service providers, faith-based organizations, organizations serving veterans, people with lived experience of homelessness, and other community stakeholders.
Rather than addressing homelessness through isolated programs, the CoC works to build a connected system of housing and services that can respond to people at risk of or currently experiencing homelessness.
Through shared data, coordinated planning, and collaboration, the CoC helps the community identify priorities, evaluate outcomes, and strengthen the overall system—with the goal of making homelessness rare, brief, and non-recurring.
